CIRM Training Grant Application Forms for RFA 05-01
The receipt date for applications in response to this RFA is July 1, 2005.
All applications must be prepared using the CIRM Training Grant Application Forms posted below. A full application consists of filling both the PDF application form and the Word form for biographical sketches. Filling the PDF form requires use of the full version of Adobe Acrobat; if you only have access to the Acrobat Reader, you may be able to print but may not be able to save your file after filling it. The Word document is the standard format for biographical sketches of Program Directors and mentors. Please use the 2-page format for each individual and duplicate the forms as needed.
Send the completed application (both PDF and Word files) as attachments to TRAINING@cirm.ca.gov. In addition, submit a signed original of the application, and the full application on a CD in one package to:
